DJ Form started as a Hip-Hop/mixtape DJ in Cincinnati, OH. Once he moved to Nashville, he hooked up with top selling Hip-Hop group, Grits. While touring with them for 9 years, he co-wrote and produced many of their top hits and was nominated for several Grammys. Soon after, Form toured as the DJ in Kelly Clarkson's band for 3 years.
