Form begun his career as a DJ at the tender age of 14, mixing Garage records. After a year of hard practice, he was recognised by a local radio station owner and became one of the youngest DJ's on the airwaves, debuting on Unknown FM.

By the age of 16, he had already played alongside some of the Biggest names in the industry at the time, including: The Dreem Teem, Karl ‘Tuff Enuff’ Brown & Pied Piper. At clubs such as Camden Palace and Gass Club.

Form widened his horizons and begun to play Hip Hop & Grime - and over the following years broadcasted on radio stations such as: Lush FM, Ouch FM, Y2K FM, Itch FM, PyroRadio & Xtreme FM! Past guests on his infamous radio shows include: Wretch 32, N Dubz, Bashy & Skinnyman.

Not just a radio personality, Form continued to DJ in clubs up and down the UK, performing alongside artists such as: Wiley, Chip, The Newham Generals & Rodney P.
